Don't trust your pre read.

If a defender backpedals with his hips square to the LOS, he's in a zone of some kind. If he turns his hips to chase a receiver or back, he's in man. If he runs at your QB it's a blitz :P

one thing ive noticed with my d and my opp. is that when the lbs are in zone they shuffle to line up with the receiver or shuffle over when someone goes in motion but when in man that one guy stays with the receiver in motion i may be wrong someone correct me if i am

Absolutely correct. Another tip...as you come out of your huddle...go to coaches cam..(zooming back out) and watch the CB's lining up with your WR's. If they jiggle a little before getting set....they are in MAN, if not...they are in ZONE.

This is how I read the defense:
1. I first read the safties. If both drop back its either Cover 2 or Cover 4. If one goes to the middle of the field its either Cover 1 or Cover 3
2. I usually always have a back or someone running a flat route. If someone follows them its man. If not its zone.

I don't read the defense I read parts of the field. Like I'll start on the left and work my way over. Or start with the flats and work my way up. Try doing that. You still have to know where your recievers will be though.

Here is how I read the defense. But it kind of depends on the sitution.
EX. 1 NO yardage in particular needed just a passing play (usually 1 and 2nd down)
1. I'll pick any passing play really it doesn't matter to me. I like to pick plays based on formation and not really what do for the most part in these situation.
2. If its a balanced pass play like 2 to routes on the left side and to routes on the right side (or 2 right 1 in the middle and 2 on the right something similiar to that) I'll look at the defense and try and guess where the better matchup is. I usually count wich side has the least defenders or best defender to reciever ratio.
3. After that I pick the deep route as my first option and a the shorter route as my 2 option(I usually only have 2 options because in madden it usually is that quick.)
4. Now I read the defense(pre-snap still). If I have linebackers or dbs look like there are about to blitz or I think they are going to blitz I imediatley flip my options to short route 1 and deep route 2.
and thats about it once I've been able to put this together with my other options my madden game has gotten crazy. These concepts are not new or bs I got the part of the simple concept from a Phil Simms passing book. Any ways on 3rd down its a little diffrent because depending on how much yards you need you have to look for and design plays to get you that yardarge. That I really can't type out because its harder to explain and its a lot of stuff(not trying to sound like I'm some kind of madden guru).
But hopes this helps
Also when its not balance just go high low with your options and remember to go low first if you think its a blitz. I telling you I never could score alot in madden before this but now I'm confident I can score a lot on most high level players on madden.
Pm if it helps or you've got questions but I haven't been on lately since I'm at school but I try to get back to you
Peace

It's easy...it only get's tricky when you're opponent freezes his D.
If you're concerned about the blitz audible out from you're current play (let's say it's I-form normal) to a spread formation like a 4 or 5 wide. defenders that follow you're guys are playing either man or zone and the one's that stay still are the ones that are blitzing.
Or 9 times out of 10 if you motion you're WR and he follows him no matter where you put him then it's man coverage. If he follows you're WR half way then it's zone (9 times out of 10).

I think reading defenses can get tricky if you're playing someone good, and who playmakers their players well, or controls the safety.
Don't rely on glitches, or regular reads or the safety's, because those will either get removed, or your opponent can throw off your reads.
I usually complete about 60-70 percent of my passes, and that's with vick, who has low accuracy, so I'm at least decent at reading defenses.
I read parts of the defense similar to what was posted above. Try to read as much defense after the snap as possible, I try to read the linebackers, and possibly the safeties if I can.
Because my first read is usually the linebackers, most of the time I progress through my reads from shortest to longest distance receiver. The only real exception to this is when I have a receiver running a streak or a fade.
From the linebackers movement, I figure out if it is zone or man coverage, and depending on which it is will usually determine who my first read is.
Say I think the flats are open, I'll progress to my first read which is usually dunn or griffith in the flats, if I see a corner covering that area of the field, and roll out with vick and hit one of my backs out of the backfield once they've passed the corner in the flats.
If I read the linebackers going into man, man/blitz, my first read will usually be crumpler.
If I see the safeties sliding, I'll look for lelie deep, etc.
I spend time in practice mode, and set up different defenses, and how I'll pass against them, and how I'll progress through my reads.
It's important to not rely too heavily on reads of defenses, because talented opponents will shut you down, and trick you with different coverages.
Go into practice mode, and set up different defenses. Depending on your initial read of the defense, decided how you'll progress through your reads of receivers.
So in essence, more time in practice mode will help you to read defenses, and figure out how to work your plays against particular defenses.
The best way to read D is to not read D.
Just read if your reciever(s) is/are open.
Madden is not rocket science. Get the ball out damnit!!!!!
